Welcome to the Montreal Children's Theatre! We are excited to have you join us for our programs and performances. Below, you'll find important forms such as our student and camper information forms, as well as our cancellation policy and media release form. Please take the time to review and submit the necessary forms and familiarize yourself with our policies.
To ensure the safety and well-being of all participants, we require the completion of the following Student or Camper Information Form for each student or camper participating in our programs. This information is vital for us to maintain accurate records and provide the best possible experience.
We understand the need for children to begin to develop their independence and therefore you may wish for your child to walk home from classes. If you wish for your child to walk home, please complete the permission slip here.
At Montreal Children's Theatre, we understand that circumstances may change, and you might need to adjust your plans. We want to be fair and flexible, so we have established the cancellation policy that you will read here.
At Montreal Children's Theatre, we believe in celebrating the creativity and talent of our participants through photos and videos. These materials may be used for promotional purposes, including but not limited to, social media posts, websites, brochures, and advertising.
By signing this form, you give us permission to use photos and videos of your child/ward (or yourself, if you are the participant) during Montreal Children's Theatre programs and events.
